How does this work?

Costs and insurances accepted

Initial assessments for individuals, couples, and familes is $150. Ongoing couples and family sessions are $125, and ongoing individual sessions are $115.  An initial phone consultation is available (no charge) to assess need and potential fit. Health insurance— with the companies listed below— may reduce these amounts significantly.    

Insurances Accepted: 

Ben: United (Optum), Golden Rule, UMR, Oscar, Aetna, Cigna (Evernorth), Banner Aetna, LyraHealth. Ben is also in network with the American Indian health Program (AIHP) Tribal ALTCS, and TRBHA.

Note:  Blue Cross Blue Shield and Anthem insurances, Medicaid, Medicare/AHCCCS health plans are not currently accepted.

Check with your insurance provider for costs as respective insurance companies and plans can differ significantly.

Sliding Scale or self-pay:  We offer a sliding scale fee based on yearly income, if desired. For individual incomes under $50k/year, we will see clients at a charge of $125 for the initial intake appointment and $85 for ongoing sessions. Couples with household incomes under $100,000 can expect to pay $125 for their initial appointment, and $95 for ongoing sessions. Families with household incomes under $100,000 can expect to pay $125 for the initial assessment, and $85 for ongoing sessions.

Ben is  well-versed in helping  people understand and accomodate so these experiences do not define you or negatively impact others.
